Student absence and COVID-19 reporting
If you will be absent from timetabled classes due to COVID-19 you must complete both of the forms below. We will replace this with an integrated reporting process as soon as we can.
To report a COVID-19 related case, please complete the form below. You can access this form using your Leeds Conservatoire username followed by @lcm.ac.uk (e.g. abc12345678@lcm.ac.uk), and your Leeds Conservatoire password.
*Please ensure that you use @lcm.ac.uk when logging in, not @leedsconservatoire.ac.uk. If you are unable to access this form, please email help@leedsconservatoire.ac.uk
You must tell us if you have been in contact with COVID-19. This means:
- If you have a positive test for COVID-19. Following a positive test, you should not come in to our buildings for five days (you can find out about staying home and avoiding contact with others here)
- If you suspect you have COVID-19 (you can check the main symptoms here)
If any of these apply to you, you must not come to Leeds Conservatoire buildings.

Staff absence and COVID-19 reporting
Please use the form to report any unplanned absence, whether COVID-19 or otherwise, and including any requests for other types of leave such as dependents leave. This form replaces the usual process of emailing HR@lcm.ac.uk (and HEAdmin@lcm.ac.uk for teaching staff). You will need to log in to your Leeds Conservatoire IT account.
